Clinical Analysis of the Gastric Cancer


Abstract
Total 597 cases of gastric cancer admitted to the Department of Surgery of Seoul National University Hospital from January 1, 1956 through December 31, 1971 has been clinically analysed.
Sex ratio was 2 : 1 male to female. Average age of gastric cancer was 51.5, most frequently developed in the 5th decade for female and 6th decade for maie(61. 6%).
The subjective symptoms were indigestion, postprandial epigastric fullness and anorexia 67%,
weight loss 42%, symptoms of pyloric stenosis 26%, mass palpable in the epigastric area 25%, anemia 15%, melena 13%, hematemesis 7%, symptoms of peritonitis 3%.
Duration of symptom to operation was average 9 months, somewhat predominated in the group of 1-3 months with 30.2%, but in the group of over 3years occupied 7%. physical examination revealed tumor palpable in the epigastric area 63%, hepatomegaly 11%, etc.
The results of gastric juice analysis were as follows; anacidity and hypoacidity 78.3% (224 cases), normoacidity 15. 3% (44 cases) and hyperacidity 6. 4% (18 cases).
Occult blood in stool was discovered in 54. 1%. 87.9% (525 cases) was operable, among¢¥525 cases of operated patients, 58.0% (306 cases, was resected. and 47.5% (249 cases) was curatively resected. In spite of mass palpable in the epigastric area resectability was 56%.
In the group of stomach cancer with pyeoric stenosis, hyperacidity and peptic ulcer like symptoms and older age, resectability were high.
On the location of tumors, 66.7% found in pylorus and antrum(350 cases), 17.2% in bqdy(91 cases) 6.6% in cardia and fundus(34, cases), 9.5% in the almost entire stomach wall(50 cases)
Incidence of gastric cancer with regional lymph node metastasis(Stage B) and invasion into the serosa(stage II) were 90.6% and 93.6 , which told us almost all cases of gastric cancer in our country was advanced cancer.
metastasis found during operation was as follows; lymph node 91%(442 cases), omentum 45%(152 cases), mesentery 23%(110 cases), liver 17%(80 cases) and pancreas 14%(68 cases).
On the 525 cases of operable cancer patients, following surgical procedures were performed; total gastrectomy 7%, partial gastrectomy 51%, palliative procedure 28%. Operative mortality was 3. 4%(18 cases).

Complications developed in 19%(89 cases) among them Complications for operative procedure itself
in the abdomen 5%, pulmonary complication 4%, intestinal :obstruction 3%, wound complication 4% occured.
Recurrence after operation lengthened in the group of longer history of initial symptom duration to the operation at the first admission.
